Renal Biopsy Case History
August, 1995

Clinical Summary

A 69 year old white male with a history of congestive heart failure and hypertension presented with the nephrotic syndrome and persistent left pleural effusion. The medical history was otherwise unremarkable except for a nocturia X3 for many years. Physical examination revealed severe edema and a blood pressure of 138/84 on anti-hypertensive treatment. Significant laboratory values included 5-7 RBC/hpf hematuria with no RBC casts, 4g/day proteinuria, BUN 29 mg/dL (10.4 mmol/L of urea), creatinine 1.8 mg/dl (160 umol/L) , creatinine clearance 48 mL/min (0.80 mL/s), serum albumin 3.2 g/dL (32 g/L), cholesterol 184 mg/dL (4.76 mmol/L), normal serum and urine protein immunoelectrophoresis, negative ANA assay, negative hepatitis B serology, and normal serum complement.

A renal biopsy was performed.
